Once your application has been completed and your application fee has been paid, please download the Application Checklist, Student Questionnaire, Parent/Guardian Questionnaire, Confidential Teacher Questionnaire, and Request for Student Records found elsewhere on this page (see the sidebar on the left).  Please follow the instructions on the Application Checklist and submit the requested information to:

The Confidential Teacher Questionnaire should be submitted to the applicant's current teacher for his/her direct submission to Unity Catholic High School. Likewise, please submit the Request for Student Records to the applicant's current school.

 Entrance Placement Exam: Saturday, January 25th
 Application Deadline: Friday, January 31, 2025
 Application Decision: End of February
 Rolling admissions continue until all classes are filled.
